coverage. All insurance premiums are deducted on a pre-tax basis,
from every paycheck, through payroll deduction. Upon termination
of employment, coverage ends on the last day of the month in which
the employee terminates.

at company expense, effective the first of the month following
90 days of continuous employment. Additional supplemental coverage
is available at employee expense.
Paid Time Off : Both full
time and part time employees earn paid time off based on hours
worked and completed service with the company. Employees
do not become eligible for vacation until the first of the
month following 90 days of continuous employment. All paid time
off is paid based on the employee’s hourly base pay rate.
All requests for vacation must be submitted to the supervisor
a minimum of one week in advance on an Absence Request Form.
Holidays : Full time employees
are eligible for holiday pay, based on their hourly base pay rate.
401k Plan : : Employees become
eligible for the 401k Plan after completing one year of service
and 1200 hours. Employees may elect to contribute up to 15% of
their base pay; the company will match 100% of the first 3% contributed
and 50% of the next 2% contributed. There are 10 investment funds
into which an employee may direct his/her funds. The plan has
3 year cliff vesting; that is, if an employee terminates prior
to completing 3 years of employment, all company match monies
are forfeited. Enrollment into this plan is offered quarterly,
following 12 months of employment.
